States Meredith can see patients: New Hampshire, Massachusetts

Patient Responsibilities:

You must be in a private location in your home or workplace. There can be no other people in the room.
You may NOT be in a car, moving or immobile (including your driveway or garage)
You may NOT be in a public location such as shopping center or parking lot
If you are running late call our office to reschedule. We allow a ten minute grace period which will not extend your visit into the next patient’s appointment.
If Meredith has not checked in after ten minutes of your appointment time call the office right away.
It is important that you are near internet and stable wifi. We encourage you sit near your wifi source inside your home. If your connection is not stable clear the cookies on your device. Ensure your internet can support the visit, if other are streaming or highly utilizing internet in your home it will affect the connection.
You must type in the full address where you are physically sitting upon check in. If you are not able to give your location, the session cannot be completed.
Do not attend your session when driving, your provider will end the session.
All HIPAA and safety rules apply to telebehavioral health. Please be sure to complete the Telebehavioral Health Consent Form and the Telebehavioral Health Safety Plan prior to your session.
